Is my hospital eligible?
AZ REACH
For Arizona's Indian Health Service, tribally operated (PL 93-638) and critical access hospitals. No prior placement attempts are required.
AZ STEP
For Arizona hospitals with 20 or fewer inpatient beds, after their normal transfer process has been exhausted. Current active period: Oct. 19, 2026 – June 30, 2027.
How it works
- Call 1-877-787-4329. Have your intake details ready.
- Your treating practitioner sets the level of care and any preferred destination.
- AZ REACH calls receiving facilities, starting with the closest available facility that has the needed specialty, and connects the practitioners.
- We follow up until placement is secured. Your hospital continues to arrange transport.
What we coordinate
- Transfers for all service lines that require acute medical care placement
- Calls to receiving facilities and practitioner-to-practitioner connections
- Follow-up on placement progress
- Reporting on your hospital's transfer trends and unmet needs
Not currently included: behavioral health placement, post-acute placement and transport coordination.
